| Individual (1 vote per individual) | Fee $50 |
| Organization 1 Membership* (a single vote, includes 2-9 members) | Fee $150 |
| Organization 2 Membership* (a single vote, includes 10-20 members) | Fee $250 |
| MPLAN Sponsor (See Below**) | Fee $400 |

NOTE: Invoice request available if required by your organization.
NOTE: Credit cards will only be processed over the phone with an MPLAN representative at a pre-arranged time. There will be a 3.5% service fee added to your payment.
*Please include or email a separate list of your organization’s individual names you would like to include for (2-9) and (10-20) members.
**The MPLAN Sponsor receives the basic Organization 2 category plus placement of their corporate name and logo on MPLAN’s website and on all event promotional materials for their active membership year.
NOTE: The MPLAN Membership year is June 1 to May 31. You can join at any time during the year. Please make cheques payable to MPLAN and send to the address below.
NOTE: For clerical efficiency, please include with your payment a copy of your membership form or a note (detailing who/what the cheque is for OR an invoice number).
